Please bring your insurance card(s) with you to your visit and notify us if any changes occur.

 

Your insurance is a contract between you and your insurance company. Not all services are covered benefits in all insurance contracts, so you will be responsible for all copayments and deductible amounts. Please remember that most insurance companies do not cover all costs. Insurance is one method of reimbursement for services provided, but does not always cover all costs.

 

Payment is due in full at the time services are rendered. We accept cash, debit cards, Apple Pay, AMEX, Discover, Mastercard, and Visa credit cards. We also accept CareCredit.
